Heavy blanket, less pain perception? American study of 94 people with chronic pain (2022)
American researchers had 94 adults with chronic pain sleep under either a weighted or a light blanket for a week. In this article, you’ll read what changed in how they experienced their pain, and what didn’t.

You get up and already feel your back or hips with the first steps. You know how the day will go: you can still do the grocery shopping, but after that you need to sit down for a while. In bed that evening, you spend some time looking for a position that feels manageable.
According to the Care Standard for Chronic Pain from 2017, developed by patient organizations and pain specialists, more than two million adults in the Netherlands experience pain every day. Researchers in the United States wanted to know whether a weighted blanket could help with that. People experience firm pressure, like during a massage, as pleasant and calming, they write, and perhaps it changes how you experience pain.
Their 2022 study followed 94 adults with chronic pain. By random assignment, half slept at home for a week under a heavy 6.8 kg weighted blanket and the other half under a light 2.3 kg blanket. There was no difference between the groups in the pain they reported each morning. However, a questionnaire about the previous week, measuring pain and interference together, did show a difference: with the heavy blanket, the score fell by about 1.4 points on a scale from 0 to 10, compared with about 0.7 points with the light blanket. The difference was greater among people who generally experienced a lot of anxiety.
So “less pain” is an oversimplification. How intense the pain felt at that moment did not differ between the heavy and light blankets. Overall pain experience, combining pain and interference, improved slightly, and what happened after that one week was not measured. Below, you can read what you can and cannot conclude from this.
Why this was studied
Behavioral therapy and mindfulness can help with chronic pain, but they require time and guidance. Pain also has an emotional side: how frightened or tense you feel affects how heavily the pain weighs on you. The idea was that the pressure of a weighted blanket, deep pressure stimulation, affects this emotional side through reduced anxiety and better sleep.
How the study worked
The study took place entirely remotely in 2020: the blanket was sent by mail, and the questionnaires were completed by phone. The participants were 44 years old on average, and 80% were women. They had various types of pain, such as fibromyalgia, back pain, migraine, arthritis, or nerve pain, and 42% used opioids, strong painkillers.
After three nights without a blanket, they had seven nights with a regular weighted blanket from a store, filled with glass beads. The participants were not told how heavy their blanket was.
Every morning, the participants moved along a 0-to-100 scale to indicate how much pain they had at that moment; this was the primary outcome. At the beginning and end, they completed a questionnaire with three questions scored from 0 to 10: how severe was the average pain during the past week, and how much did that pain interfere with your enjoyment of life and daily activities?
What the researchers found
The pain at that moment did not differ between the groups. In one week, the morning score fell from 56 to 43 with the heavy blanket and from 56 to 49 with the light blanket, but this difference could be due to chance.
The questionnaire score fell in both groups, more with the heavy blanket: after adjusting for expectations and anxiety, by about 1.4 points compared with 0.7 points with the light blanket. According to the researchers, this difference was probably not due to chance. When they looked only at the two questions about interference, the difference was not clear.
People who generally experienced a lot of anxiety benefited more from the heavy blanket; among those with little anxiety, the researchers found no clear difference. 56% of participants were above the threshold at which the difference became clear. This analysis was not included in the plan in advance.
Anxiety and sleep did not improve with the heavy blanket, so contrary to expectations, they did not explain the effect.
What this study does not say
The primary outcome, pain at that moment, showed no difference; the positive result came from the questionnaire, an additional measurement.
The study lasted one week; we do not know what happens afterward.
At the end, many participants guessed which blanket they had: 62% in the heavy group and 68% in the light group. This may have influenced their assessment.
Of the 118 participants initially assigned to a group, 16 dropped out, 14 of them from the light group. Another 8 were not included in the analysis.
The participants reported their own pain; a doctor did not verify it.
This study says nothing about a weighted blanket compared with no blanket at all, because the light blanket also provided some pressure.
How this fits with other research
As far as we know, this is the only randomized study of weighted blankets for chronic pain. We do not know of a study involving only people with rheumatism or fibromyalgia. People with fibromyalgia and arthritis did participate in this study, but the researchers describe the outcomes by type of pain as “very preliminary.” Another American study found no difference in pain just before surgery (see weighted blankets in the hospital). For insomnia, a Swedish study using similar weights did find a clear effect on sleep.
A larger study is now underway: the U.S. Department of Veterans Affairs (VA) is comparing a heavy and a light blanket for six weeks in 160 veterans with chronic muscle and joint pain and sleep problems (NCT05963607). According to the trial register, recruitment is still ongoing and the primary measurements will be completed in April 2029. For the bigger picture, see the overview of all the research.
Study at a glance
Researchers: Jennifer Baumgartner, Laura Case and colleagues (UC San Diego)
Year and journal: 2022, The Journal of Pain
Country: United States
Study type: randomized: random assignment determined who received which blanket
Participants: 94 adults aged 19 to 69 with chronic pain, 47 per group
Duration: one week, after three nights without a blanket
Blanket: weighted blanket with glass beads, 6.8 kg versus 2.3 kg
Do you have chronic pain and want to try a weighted blanket? This study did not find that pain at that moment became less intense than with a light blanket; at most, overall pain experience improved slightly, especially among people with a lot of anxiety. A blanket is not a substitute for treatment. If you take pain medication or have another condition, consult your doctor or healthcare provider first.
In the study, everyone had to be able to lift 6.8 kg safely, and at home you must also always be able to remove the blanket yourself. To find out which weight is right for you, read the article about choosing the right weight, and learn more about safe use on the safety page.
